The Detroit Community Action Plan |
In 2000-2001, members of the REACH Detroit partnership in the Detroit area developed a Community Action Plan to address the problem of diabetes and related conditions. The partners spent a year meeting with people in the community, many with diabetes, and developing the Community Action Plan.
This detailed plan, with a focus on preventing diabetes and its complications in African Americans and Hispanics, is now being implemented. The Community Action Plan consists of four interrelated interventions. Click on the interventions below to see their goals and activities:
